A brief guide on how to apply for participation in the National Health Insurance, Labor Insurance, and Retirement Benefits (Labor Pension) schemes in Taiwan.
For a company with 5 or more employees, you must apply for participation in the National Health Insurance, Labor Insurance, and Retirement Benefits (Labor Pension) schemes. For a company with less than 5 employees, you need only apply for participation in the National Health Insurance and Employment Insurance schemes. Of course, even if your company has less than 5 employees, you can still opt for participation in the Labor Insurance and Labor Pension schemes. The forms are downloadable below:

Once you fill in the form, you should post it to your local Bureau of Labor Insurance branch. The addresses of the local branches located throughout Taiwan are as follows:
